The scope of services includes flexible individualized treatment, rehabilitation, supervision, and support that vary in intensity, depending on youth and family needs. The services are delivered by a core team of staff using a Wraparound Philosophy support which helps family, support members, and service providers work together to support the family or child. In addition, all services are delivered using a Trauma Informed Care approach. The Outreach Counselor will provide an average of three to four hours of direct support weekly to each assigned family. Services will be tailored to meet the unique needs of each youth and family served, providing opportunity to maximize their ability to spend time together at home and in the community. Responsibilities include: To provide advocacy and outreach services to assigned caseload of youth and their families, in individual or family's home and community To monitor Individual Treatment Plans (ITPs) for all youth and families that supports their capacity for growth and change To establish and set appropriate limits and boundaries as a practitioner in order to assure the safety and protection of the family members and maintain the helping role of the practitioner To provide support and structure to individual and families while monitoring their progress towards completion of service goals To transport individuals in a safe manner in matters related to service delivery To provide on-call emergency coverage as assignedQualificationsHigh School Diploma or equivalent.
